What does hydroxychloroquine combined with methotrexate do for ra

I've stopped Sulfa as I was experiencing a bad taste & smell, which may be caused by sulfa. After a while it wasn't as effective and I too was prescribed methotrexate alongside. I'm about to start MTX & am hoping that I wont have side effects with that! The side effects of the methotrexate for me were horrendous (chronic chest infections) and I'm now asthmatic as a result.

I was diagnosed with RA in march and have been on increasing dose of methotrexate. Yesterday my rheumatologist started me on hydroxychloroquine as well. Is anyone else on this combination and have you had any problems with it as I know hydroxychloroquine can cause eye sight problems. Plaquenil was actually first classified as an antimalarial drug, but it is also used to treat certain rheumatic and autoimmune conditions which are unrelated to malaria. Generally, Plaquenil is a treatment option as monotherapy used alone for mild rheumatoid arthritis or as combination therapy with other DMARDs for moderate to severe disease. Methotrexate tinkers with the immune system, but it doesn’t suppress it in the same way that some other RA drugs do. So taking methotrexate is rarely associated with increased infection risk unless you’re also taking other medications at the same time, such as glucocorticoids, immunosuppressive disease-modifying antirheumatic drugs DMARDs, or biologics.

I had side effects from MTX, which largely wore off, but haven't had any problems with the Hydroxy or Sulpha. I refuse to take it now even if its my passport to biologic treatment. I started with MTX which worked, but not enough so the others were added in which has been better.

  5. The study showed that the combination of methotrexate and hydroxychloroquine is equivalent to leflunomide in improving the disease activity and is similar in the hepatic safety profile. We conclude that methotrexate in combination with hydroxychloroquine is equal in efficacy to new drug leflunomide as an initial therapy in severe active rheumatoid arthritis.

Hydroxychloroquine is a disease-modifying anti-rheumatic drug DMARD approved for adults with RA. It is often given in combination with methotrexate and sulfasalazine a treatment known as triple therapy, to help slow RA disease progression for patients who do not get relief from methotrexate therapy alone. CONCLUSION Combined basic therapy of RA with methotrexate and plaquenil was more effective than monotherapy with methotrexate because it produced good effects more frequently and earlier while no response was seen less often. Methotrexatehydroxychloroquine combination therapy may cause lower disease activity in early RA than methotrexate monotherapy. Methotrexate monotherapy as first early RA treatment seems to lead to more treatment intensification.
