Baseline reading for plaquenil therapy 2018

What tests should be done at the six month interval? Certainly, I have had no problem whatsoever from the plaquenil..even knowing plaquenil induced eye problems are rare...

The American Academy of Ophthalmology released an updated set of screening recommendations for hydroxychloroquine Plaquenil and chloroquine to account for the many studies that have shown the effects of these medications on the retina 1. It succinctly makes the case for screening, and The guidelines also redefined the various categories of hypertension. It eliminated the category of prehypertension, which had been defined as systolic blood pressure of 120 to 139 mm Hg or diastolic pressure the lower number in a reading of 80 to 89 mm Hg. I was told from the start by my rheumatologist I would have to have eye exams including visual field exams every six months due to the plaquenil therapy. My first two exams did include these tests. But my next six month appointment is coming up, and they told me I didn't need the visual field test this time so the appointment would be shorter.

I am also on low dose prednisone, I have taken prrednisone daily for 18 months at levels ranging from 2.5 mg to 5 mg...periodic boosts to 60 mg with a fast taper back down to 5 mg. I would like to make sure they continue to follow the proper guidelines.

Newer guidelines state that daily dose 5mg/kg of real weight/day can lead to toxicity. Retinal toxicity is irreversible and can progress after cessation of hydroxychloroquine, thus early screening is important to limit potential vision loss. Baseline screening and annual screening after five years is recommended. A baseline ocular examination is recommended within the first year of starting PLAQUENIL. The baseline exam should include best corrected distance visual acuity BCVA, an automated threshold visual field VF of the central 10 degrees with retesting if an abnormality is noted, and spectral domain ocular coherence tomography SD-OCT. Tests, at baseline and periodically during treatment. Institute medical management promptly to include specialty consultation as appropriate. In general, withhold LIBTAYO for Grade 3 or 4 and certain Grade 2 immune-mediated adverse reactions. Permanently discontinue LIBTAYO for Grade 4 and certain Grade 3
